What is social prescribing?

We could help if you:

  • Feel socially isolated (don’t have anyone to talk to)
  • Want to improve your physical or mental health
  • Lack confidence
  • Need to find some practical support and information to improve your situation, and/or how you feel

Our team will:

  • Work with you to find activities and community services that suit your interests and needs.

How can a social prescriber help?

We can help if you:

  • Feel isolated
  • Want to improve your physical and/or mental health
  • Lack confidence
  • Need practical support to move on from something that’s affecting your health and well-being, for example, financial difficulties or a bereavement.

We’ll work with you to find activities and community services that match your interests and needs.

We can help to:

  • Find clubs and activities for children and young people
  • Find support groups and courses for under 18s and parents
  • Support with mental and physical health conditions

And much more.

How are we funded?

For over 18’s support our social prescribers sit in Flintshire Council’s Single Point of Access (SPOA). For family support our social prescriber sits on Flintshire Council’s Early Help Hub.

We are funded through our partnerships with Betsi Cadwaldr University Health Board, Integrated Care Fund, Flintshire County Council and Families First.
